Background: Sakta (stroke) is described as a sudden loss of sensation and voluntary movement due to interruption of the vital pneuma (Ruh Dimāghī) in the brain, When caused by vascular blockage due to thick Balgham (phlegm) or neuronal injury, it resembles the modern concept of ischemic stroke (Sakta Dimaghi Inside) and a major cause of morbidity and mortality worldwide, In Unani medicine, stroke-like conditions are described under the term Sakta or Falij-e-Nisfī, attributed to derangement of Balgham and obstruction of Rooh Dimaghia Unani scholars such as Ibn Sina and Razi emphasized the role of humoral imbalance and disturbance in Tabiat (innate healing power) in its pathogenesis.
Aims: To evaluate the clinical efficacy and safety of Unani pharmacotherapy combined with regimental therapy in post‑stroke recovery.
Intervention: A clinical case report was conducted in the Department of Moalejat, Ajmal Khan Tibbiya College & Hospital‑A.M.U, enrolling two eligible patients. The treatment protocol included Munzije Balgham administered once daily in decoction form for 21 days, followed by Mus’hil Balgham on the 17th and 21st days. Massage therapy with Rogan Qust, along with parallel walking bar exercises, was prescribed from the 21st day through the 90th day. A phlegm‑reducing diet was also advised throughout the treatment period.
Outcome: After twelve weeks of therapy, marked improvement was observed in motor strength and very slight improvement in speech not facial deviation. The patient regained slight independent mobility with minimal residual weakness
Conclusion: This case demonstrates that Unani treatment, based on humoral balance and deobstruction of cerebral vessels, can effectively support recovery in ischemic stroke. Further clinical research is needed to substantiate its therapeutic value.
