Effects of Aftimoon vilayati (Cuscuta europaea) in the Management of Izterab-i-Nafsani (Anxiety): A case report

Abstract:
Introduction: The word anxiety, which includes behavioral, affective, and cognitive reactions to the sense of risk, comes from the Latin "anxietas", which means to choke, throttle, trouble, and upset. It is common human emotion when it occurs under the challenging situations of life. There is no explicit description of anxiety in Unani medicine instead; Fikr (worry) is used as a synonym for anxiety. In Unani medicine, anxiety is referred to as Izterab-i-Nafsani, which stands for worry, excessive thinking and fear. It is also used to describe the feeling of impediment in routine work.
Clinical presentation: A 38 year old male patient, Diabetic, Hyperpertensive and Euthyroidic came to the OPD, Department of Moalajat, Regional Research Institute of Unani Medicine, Habak, Srinagar with the complains of feeling of increased nervousness and sense of fear. When the patient approached to Outpatient department, he had already been diagnosed with anxiety and was put on medications. The patient complained of getting the anxiety attacks once in two weeks which affected his day today life. Further patient also companied of hot flashes and palpitations which would increase whenever patient had to encounter a stressful condition.
Intervention: Aftimoon vilayati (Cuscuta europaea) which was identified by the botanist was given in the form of decoction with dose of 5 grams twice a day along with life style modifications and dietary changes.
Results: The signs and symptoms of the disease showed a significant improvements were evaluated pre and post treatment with the help of scales like Hamilton Anxiety Rating Scale (HAM-A), Beck Anxiety Inventory (BAI) and Visual Analog Scale for Quality of life.
Discussion: The results obtained in this case can be attributed to the fact that Aftimoon vilayati given to the patient along with the life style modifications and dietary changes helps to combat the signs and symptoms of anxiety through the evacuation of Sawdā (black bile) considered to be responsible for causing anxiety and through the anxiolytic activity of the drug present due the occurrence of methanol and flavinoids in Aftimoon vilayati.
Conclusion: From the results and discussion it can be concluded that Unani system of medicine provides a good alternative option to manage diseases like anxiety and depression.
